Get started with PSM Forms
In an effort to save you time on your first visit, you may access our New Patient Forms here to print and complete them prior to arriving at our office. Please fill in the New Patient Form and Medical History completely, review the Office Policy and initial each section, and sign either the Acknowledgement or Waiver section of the HIPAA Notification Form. Please bring all forms with you at the time of your first visit, along with your insurance card and your doctor's referral, if you have one. Please note that some insurance companies require additional paperwork which we will provide in the office.

Having these forms completed will help expedite your check-in process, however please arrive 10 minutes prior to your appointment time so the front desk staff can review your insurance benefits, our office policy and complete your chart set-up prior to beginning treatment with your therapist.

As of April 14, 2003 it is required for all patients to understand their rights under the Health Insurance Portability and Accountability Act of 1996.  To comply with federal regulations, you may view our HIPAA Privacy Notice online or in our clinic.
