Review the instructions below to optimize your first visit with us.

Follow this link to our New Patient Forms. Please fill in the New Patient Form and Medical History completely. You may fill the forms directly in Acrobat Reader and print from your computer. Please bring these two forms with you at the time of your first visit. Having these forms completed will help expedite your check-in process, however please arrive 10 minutes prior to your appointment time so the front desk can review your insurance benefits, our office policy and complete your chart set-up prior to beginning treatment with your therapist.

As of April 14, 2003 it is required for all patients to understand their rights under the Health Insurance Portability and Accountability Act of 1996.  To comply with federal regulations, you may view our HIPAA Privacy Notice online or in our clinic.
