Getting Started

Welcome to Presidio Sport & Medicine!  We look forward to the opportunity to work with you. Scheduling Appointments: When you are ready to schedule an appointment, please give us a call so we can best serve you. For scheduling, it will be helpful for you to have your calendar available.  If you are scheduling a physical therapy evaluation and billing insurance, you will also want to have your insurance information and doctor’s referral available.  We recommend clients schedule appointments well in advance in order to secure their preferred days and times.  If we do not currently have an appointment time that best meets your schedule, we offer a waiting list and will do our best to accommodate your needs.

Your First Appointment: To prepare for your physical therapy initial evaluation, we ask that you call your insurance company to verify your outpatient physical therapy benefits, bring with you your referral for physical therapy and your insurance card, and allow 15 minutes prior to your scheduled appointment time to complete necessary paperwork.  If you would like to complete the New Patient Forms prior to coming to our office, you may refer to our  New Patient Forms page.  Please wear clothing that is comfortable to move in and allows us access to your injured area.  For your initial evaluation appointment, you should plan to be in the office for approximately an hour and a half.  For directions to our facility, please refer to our Driving Directions page.

Confirmation Calls: As a courtesy, we offer an automated confirmation system which will place a reminder call or email to you two days before your scheduled appointment. Please be aware that you are responsible for knowing your scheduled appointments and should not rely solely on courtesy confirmations.

Cancellations: Appointments you schedule are reserved especially for you. If you need to reschedule or cancel an appointment, we request and appreciate a minimum of 24-hours’ notice. For the best customer service, we ask that you make schedule changes during our normal business hours. Appointments cancelled with less than 24-hours’ notice will be assessed a fee of $125 for physical therapy evaluations, $50 for physical therapy appointments, and the full price for massages and fitness services.

Insurance: We accept most major insurance plans.  For more information on which companies we are contracted with, please refer to our Insurance & Billing page. If you are utilizing your insurance for physical therapy services, we will submit claims on your behalf.  It is important to contact your insurance company prior to starting physical therapy to verify your outpatient physical therapy benefits.  We will do our best to help you understand your benefits.  Since we are not your insurance company, we cannot guarantee eligibility, extent of coverage or any promise of payment by your insurance.

Payments: In an effort to keep our fees and your balances low, we ask that you make payment at the time of service.  Please check-in with our Client Service Specialists prior to each appointment and allow a few minutes to complete payment prior to your services.  If utilizing your insurance, you may have further financial responsibility as designated by your insurance company after your claims have been finalized. You are ultimately responsible for any outstanding balance.
